Rental Booking Form Thank you for choosing our rental services! Please fill out the form below to reserve your boat. Acknowledgment of Liability By signing this form, I acknowledge and agree to the following: I assume full responsibility for the boat, equipment, and all passengers during the rental period. I agree to follow all safety guidelines and boating regulations provided. I understand that any damage to the boat, equipment, or third parties caused during my rental period is my responsibility and may result in additional charges. I release and discharge [Company Name] from all liability, loss, claims, or damages that may arise from the use of the rented boat, except in cases of gross negligence by the company. I confirm that I and any additional operators of the boat are at least 18 years of age and possess the necessary qualifications and/or boating licenses (if applicable). Cancellation Policy Cancellations must be made at least 48 hours in advance for a full refund. Cancellations within 48 hours may incur a fee.
